ABSTRACT

In an era where many advancements and accomplishments in information technology over the past decades has been tremendous, health and healthcare delivery has not been left out; quite a number of diverse forms of Electronic Information Systems have been designed and implemented in different institutions and countries. The field of health care has been transformed globally as a result of Information and communication technology (ICT). The need of a system that can aid individuals store their own health records is on the ascending. The personal storage of one’s health-related data is termed as Personal Health Record (PHR) (Roehrs, Da Costa, Da Rosa Righi, & De Oliveira, 2017). It is has become universal to access your personal information of all types online, from academic grades, to financial and banking statements, bills, and profiles, just to identify a few. One major form of data/information that has been slow to adaption to the digital environment is personal health record (PHR). It is gathered that most individuals do not possess any form of personal health record system, some rely on the manual form of storage where some hard copies of hospital reports, x-rays and laboratory results are kept at home in a paper file or some folder or cupboard. The development of PHR system is to draw attention to the need for personal health record system. In the developing process, the SDLC was followed. Requirements was gathered through interview, analyzed and used to identify the processes the system would go through. The development platform was chosen based on the available time for coding, debugging, testing and deploying. An application that runs in a web browser environment was developed relying on a localhost system to support the database system.
